Case Manager
As a Case Manager, Sue is deeply compassionate, helping individuals and families move through the needs and challenges that arise. Her goal is to promote safety, quality of care, and cost-effective outcomes. Sue works with families to helps break down barriers and better prepare both the client and their family for life both during and after treatment.
Sue also works with wraparound teams and treatment organization that manage specific care for individual clients. The treatment plan is collectively designed by the client, the client’s clinical team and the coach (me) to foster a co-active coaching relationship.
She is deeply committed to the collaborative process and helping individuals and families move through the needs and challenges that arise. Her goal is to help reduce the burden on family members by overcoming the day-to-day details that emerge during care, as well as better prepare them for life during and after treatment. This includes facilitating discussion of family roles and identifying ways to improve communication and rebuild trust.
In Sue's role as case manager, she is always cognizant of her responsibility to promote safety and deliver quality cost-effective outcomes. Part of the treatment plan includes helping patients develop a relapse prevention plan and connecting them to outside support groups that can provide resources, support and crisis intervention services when issues arise.
